    Canada. Commission of Inquiry on Aviation Safety: The Commission of Inquiry on Aviation Safety was established by Order in Council on 3 August 1979 (PC 1979-2122) under Part 2 of the Inquiries Act to investigate and to report upon the existing state and management of the Air Administration of Transport Canada pertaining to six subject areas. These were: inspection and certification of aircraft; the conditions under which aircraft may be used or operated; the formulation and enforcement of laws, regulations and rules necessary for the safe navigation of aircraft; accident investigation; the personnel involved in the above mentioned activities and any related matters. The Honourable Charles L. Dubin of the Ontario Supreme Court was appointed Commissioner. The Commission's work was therefore divided into six distinct phases: air worthiness; accident reporting; enforcement of regulations; navigational aids; uncontrolled airports and personnel. During each phase hearings were held in various locations across Canada.
