Online requests and forms

To make a formal request under the Access to Information Act or the Privacy Act, you must be a Canadian citizen, a permanent resident of Canada, or an individual or corporation currently present in Canada.

Formal and informal requests: What is the difference?

Formal requests are those made under the Access to Information Act or the Privacy Act. There is a $5.00 application fee for requests made under the Access to Information Act. There is no fee for requests made under the Privacy Act. For all formal requests, a response is required within 30 calendar days of the date the request was received.

Informal requests are not covered under any Act. There are no application fees for informal requests. The number of requests received and the complexity of each request dictate the timeframe for the completion of informal requests.

Formal request under the Access to Information Act

  • A $5.00 application fee applies to requests made under the Access to Information Act. This fee can be paid online by credit card or Interac Online, using the payment page, after you have submitted your request. Requests submitted without the $5.00 application fee will not be processed until payment is received, and will be considered abandoned after thirty (30) days.
  • Your request must include a description of the requested records. Please provide the following information:
    • a brief description of the records requested, or the research question; and
    • LAC archival references, if available. If you require, please contact LAC's Reference Services Division.
  • To make a formal request for military service files under the Access to Information Act
    • Please include in your request the following information about the individual in question: last name, all given names, date of birth, place of birth, date of death (if applicable), place of death (if applicable), service number, rank, approximate dates of service, and the individual’s parents’ names. This information is used to positively identify the correct file.
    • Please include a signed authorization form if the individual is still living, or proof of death and proof of relationship if the individual whose records you are requesting has been deceased for fewer than 20 years. If the individual has been deceased for more than 20 years, only proof of death is required.

  • Your request must include a description of the records requested. Please be as specific as possible and provide all relevant information to help us locate the records.
  • Your personal information request must be signed. Please be sure to complete, sign, scan, and upload the relevant form below with your online request:

    • Please include the following information in your request: last name, all given names, date of birth, place of birth, service number, rank, approximate dates of service, and the individual’s parents’ names. This information is used to positively identify the correct file.

Formal request for personal information under paragraph 8(2)(k) of the Privacy Act

  • Please include the exact archival references to the restricted archival federal government records you wish to access. If you require assistance, please contact LAC's Reference Services Division.
  • Please attach the relevant Band Council Resolution (BCR) or letter authorizing you to access and conduct research on files pertaining to First Nations or Inuit association research. The BCR must include:
    • the name of the researcher or the research firm. If a research firm is named in the BCR or letter, please include a letter or other evidence from the firm proving your association with that firm;
    • the name of the reserve, band and the province or territory;
    • the specific nature of the research relating to the claim; and
    • the signatures of the Chief and Council (quorum).

  • To locate a personnel file, we require the employee's surname, all given names, date of birth and social insurance number (SIN).
  • If you do not know the date of birth or SIN, secondary information (e.g., names of next of kin, department where employed and dates of employment) can assist in identifying the correct individual.
  • If you are requesting your own records and you left the public service less than two years ago, please contact the Human Resources unit of the department where you were last employed.

  • To locate a file, we require surname, all given names, date of birth and/or service number or social insurance number (SIN).
  • If you do not have that information, secondary information (e.g., names of next of kin, postings, dates of service, or place of enlistment) can assist in identifying the correct individual.
  • Please specify the documents you require. If you are doing family history research, we recommend that you request a "genealogy package," which will include copies of selected documents from the file that highlight/summarize the individual's service.
